The provisional admission is liable to be cancelled if they fail to submit documents required by the University. For Collection of Admission Form: 1. Original Bank Challan 0f Online Application. 2. Original Mark Sheet of Qualifying Examination [HS (10+2) or equivalent] For Departmental / Office Verification: 1. Original Mark Sheet 2. Proof of Residence -Original Ration Card / Voter's Identity Card / Passport. 3. Proof of date of birth in Original 4. Original Caste, tribe or disability certificate from competent authority 5. Domicile certificate, if applicable. 6. Affix Passport Size Recent Photograph on Admission Form Attested photocopy of these documents (except four Copies of passport size photograph) must be attached with the Admission Form before verification. For Admission Form Submission to the College Office: 1. Verified, Completely Signed and Fees Paid Admission Form with all attested attachments etc 2. Fill-in Calcutta University Registration Form 3.
