Notification for students who have not yet received their Identity Card.

It has come to our notice that some in-campus students of Gauhati University have not yet received their Identity Cards. Therefore, all such students who have not yet received their Identity Card are hereby directed to submit their details through the Google Form link provided below:

Important Instructions:

• Upload a clear passport-size photograph (File type: Image; Maximum file size: 500 KB).
• Upload a clear image of your signature with a transparent background only.
• In the “Permanent Address” field, provide the following details only:
  • Area/Village Name
  • Street Name (if any)
  • Town/City Name
  • District Name
  • State Name
  • PIN Code

NB:

• The submitted data will be verified against the existing printed Identity Card records. As such, students who have already received their Identity Cards are strictly advised not to fill out this form.
• The deadline for submission of the Google Form on the above-mentioned link is: March 25, 2026, (upto 11:59:00 PM)
