Items to be submitted during admission

Those who will be selected for the final admission will be required to submit the following:

• Fill up the Google form: https://forms.gle/A2i5SszHefm55LA79
• One self attested copy of MARKSHEET and the original marksheet of H.S. or equivalent examination. (Laminated marksheet will not be accepted)
• One self attested copy of ADMIT CARD & MARKSHEET of Madhyamik or equivalent Examination.
• A conduct and character certificate from the head of the institution last attended.
• Two recent passport size photographs of the student in white dress (unattested).
• A Health Certificate by a registered medical practitioner with blood group.
• Original School leaving Certificate
• A Caste certificate (wherever applicable).
• Those who fall under the EWS (ECW) category must submit EWS (ECW) certificate. The certificate is valid for one financial year and is issued by the appropriate authority (District Magistrate or Additional District Magistrate/BDO/SDO).
• Aadhaar Certificate
• An income certificate of the guardian from competent authority (BDO/SDO/any Gazetted officer).
• All original documents of the above are to be produced.
