• Payment Chalan or Bank Chalan. (Original)
• Proof of Date of Birth. (Original & Copy)
• Mark-sheet and admit card of the last Qualifying (10+2) examination. (Original & Copy)
• SC/ST/OBC-A/OBC-B/Disability Certificate (if applicable). (Original & Copy)
• One copy of recent passport size photograph
