• Payment chalan or Bank chalan Original.
• Proof of Date of birth Original and copy.
• Mark-sheet and admit card of the last Qualifying (10+2) Examination Original and copy.
• SC / ST / OBC-A / OBC-B / disability Certificate (if applicable) Original and copy.
• One copy of recent passport size photograph (if applicable).
